Hair Transplantation Treatment History

The history of surgical hair replacement in humans originates in Asia,

  • in the writings of the Japanese physician Shoji Okuda (1939) the results of the operations performed by them are described. In Okuda's notes, you can find almost all modern provisions of hair transplantation, including the principle of donor dominance. However, Dr. Okuda performed 1–5 mm grafts exclusively in cases of cicatricial and congenital alopecia and did not use it for androgenetic hair loss. In addition, all records were kept in Kanji (ancient Chinese iroglyphs), and only in 2003 a full-fledged English translation of his works became available.
  • Another Japanese pioneer, Professor Tamura, applied the single-hair graft technique back in 1943 and concluded that the smaller the grafts, the more natural the result. Unfortunately, the Second World War prevented the whole world from learning about the successes of Japanese surgeons.
  • Hair transplantation moved to the next stage only in 1959 thanks to Dr. Norman Orentreich. He used 6–12 mm punchy to isolate grafts and transplanted hair for androgenetic alopecia, due to which he is considered the pioneer of this trend. Since Western doctors did not know the work of their Japanese colleagues, the development of surgical hair restoration again went a long way of trial and error.
  • In 1980, Headington first described the concept of a follicular unit. This histological structure includes 1 to 4 hair follicles along with the adjacent sebaceous gland and the hair lifting muscle. Being located at the same distance from neighboring groups, the units are clearly visible at a small magnification.
  • In 1995-2000. American surgeons Birnstein and Rossman formulated a new concept of hair transplantation. According to the innovative doctrine, follicles should have been transplanted precisely in the form of follicular associations (units), i.e. in the same form in which they grow in their natural environment on the scalp. The follicular unit (or graft) is the morphofunctional element of the scalp. It can be one, two, three, four-haired. Later, the same surgeons developed the FOX procedure technique, now known as FUE (or seamless method), during which units are removed directly from the skin using punches up to 1 mm in diameter.

So, what is Hair Transplantation?


Hair transplant is the transplantation of hair grafts from the donor area to the recipient - the hairless area. The recipient area can be thinned or completely bald.

The success of hair transplant depends on:

  • reasons contributing to the development of alopecia,
  • the state of the donor zone (the best donor zone is the back of the head),
  • the patient's circulation,
  • care of transplanted grafts.

According to the method of follicle extraction and planting, there are 3 types of hair transplant:

  • FUE (Follicular unit extraction) is a method in which hair grafts are extracted one by one from the donor area and transplanted into incisions in the recipient area.Sapphire FUE -  focuses on the channel opening stage of the treatment, through the use of specialist sapphire — rather than steel — blades.
  • DHI (Direct Hair Implantation) is a method in which hair grafts are taken from the donor area one by one and transplanted directly into the skin without preliminary incisions.
  • FUT / STRIP (Follicular Unit Transplant) - involves the cutting of a strip from the scalp and extracting individual follicles out of it.

Hair transplant is performed on:

  • scalp
  • brows
  • beard / mustache.

Hair Transplantation for:

  • Those with balding or thinning hair.
  • Those with receding hairline, sparse eyebrows, and beard / mustache.
  • Those with traction alopecia.
  • Those who have lost their hair due to head, eyebrow or beard trauma.

Hair Transplantation Duration:

  • Hair transplantation usually takes 4-8 hours.
  • Total treatment takes 2-3 days.

Hair Transplantation Recovery Timeline:

  • Return to light daily activities after two days.
  • Return to the office in a week.
  • Redness and scabs should disappear in 2-3 weeks.
  • After 10 days, you can wear soft hats and baseball caps.
  • You should wait at least four weeks before putting on your helmet again.
  • Exercise, including swimming, one month after treatment.

Hair Transplant Healing Process

Healing can take somewhere between 6 and 12 months. 

During the first two weeks after hair follicle transplant, the donor area and the transplant site are slightly sore. It is also likely that you will see visible scabs, but this should gradually improve over the second week. After 10-20 days, hairs will fall out, this is a completely normal part of the process, and they will resume in the coming months.
Most probably, up to 90% of your transplanted hair and possibly some of your natural hair near the donor site will fall out. This occurs in 95% of hair transplant cases and is a natural, temporary response to treatment. After that, the hair will gradually recover and grow back. Some clinics may also recommend a course of platelet-rich plasma treatment to improve and stimulate this recovery.
Between the second and fourth months after treatment, your hair may grow rather slowly, usually around 5%. However, by the fourth month, you should notice that about 20-25% of the new transplanted hairs are starting to grow. By the sixth month, you should see about 40-50% new hair growth.
Between the sixth and ninth month, your transplanted hair should grow by about 75%. After the ninth month, your transplanted hair should continue to grow, usually at the last 25%. It is worth noting, however, that in some cases it may take up to 18 months after surgery to completely restore the hair at the crown of the head.

Hair transplants can be divided into three major groups: In FUT method, healthy hair roots on a complete strip of skin are taken from the donor area and transferred. In the FUE method, on the other hand, channels are opened, and hair follicles are transferred individually. DHI is literally the same method with FUE, the only difference is that harvested grafts are implanted directly into the skin without making incisions. FUE and DHI are most preferred methods nowadays. With these methods, there are no wounds or scars in the donor area, the tiny holes heal very quickly and the patient does not experience any pain.

Currently, the FUE method is considered the best. FUE is the abbreviation for Follicular Unit Extraction. The method is not as invasive as the traditional transplant method, does not leave scars, and does not require stitches. This method also allows to transplant the maximum possible grafts - up to 5.000.

The hair transplant success rate is largely related to the expertise of the medical staff. When performed by a medical team with considerable experience, the success rate is at 98%.

The results of a hair transplant are clearly visible and are considered to be permanent for a lifetime.

Alopecia is a generalized term for different types of hair loss: from hair loss in certain areas to complete baldness and, in extreme cases, hair loss on the face and body.

An Eyebrow transplant is a surgical method to restore the hair of the Eyebrow. It is used for progressive alopecia of various types, as well as for hiding scars, wounds, burns, and the consequences of unsuccessful plastic surgeries on the face. Hair is collected from the donor area, which is located on the back of the head between the ears. Then, depending on the method, incisions are made into which the hair is transplanted (FUE) or transplanted without incisions directly under the skin (DHI). The world standard for hair implantation is considered to be at least 90%.

FUE - Follicular Unit Extraction. The most preferred method for hair transplant. With this technique, the grafts in the donor area are collected individually, then the channels for future hair are opened and the hair is transplanted into the recipient area one by one. This technique allows you to take the largest amount of donor hair and minimize the percentage of rejects when removing the grafts. The FUE technique is painless.

DHI Direct Hair Implant. DHI hair transplant is performed using a medical instrument that looks like a pen. As with the FUE method, the grafts are removed first, but unlike FUE, there is no need to open the canals for them, they are placed directly into the recipient's area using the implanter pen.

FUT - Follicular Unit Transplantation. In the FUT method, hair follicles are removed by removing a strip of scalp 15-20 cm to 2 cm from the back of the head, between both ears. The hair follicles are then harvested from the strip and individually transplanted into the recipient area. The area where the strip of skin was removed is sutured.

The FUE method is suitable for people who need to transplant a large amount of hair onto large areas of the head, beard or eyebrow. It is possible to transplant the maximum amount of hair like this, up to 5 thousand hairs. The DHI method is ideal for people with small bald patches who need to thicken their hair a little, as this method allows only up to 2 thousand hairs to be transplanted.

The hair for the transplant is taken from the patient himself, as the hair of another donor do not take root. Hair from the back of the head in the area between the ears is suitable for transplantation. This hair is the least prone to shedding. If there is a shortage of donor hair, you can also take hair from the beard under the chin. Some specialists also take body hair, but this hair is fickle and can fall out after 2-4 years, unlike hair from the back of the head, which takes root forever. In addition, when collecting hair from the body, small scars may remain on the skin.

You’re a good candidate for Hair Transplantation if you have had hair loss or trauma or have thin hair naturally or as the result of a medical condition, like hypothyroidism, if you are generally healthy and have realistic expectations.

Turkey is one of the most preferred countries for hair transplantation, followed by Poland, Hungary, Thailand and Germany.

Although hair transplantation can be performed on any patient over the age of 18, doctors agree that transplantation is not advisable until the patient reaches the age of 25. At this age, hair loss begins to stabilize.

Hair transplantation does not require aftercare in literal sense, but there are some points that you should pay attention to. For example, you should avoid alcohol and cigarettes for a week before and after the operation, sleep only on your back for minimum one week to avoid excess swelling, wash your hair only with the special shampoo you will receive from our clinic for a month, and avoid heavy physical exertion for the first 2 weeks after the transplant. Your attending physician will talk with you about further details.

Within 2 to 3 weeks after the procedure, the transplanted hair will fall out. But do not worry, this is completely natural! The transplanted hair is broken and therefore falls out. But the transplanted roots are not affected by this!

After 2 to 4 months, the new hairs start to grow. Within 6 months 60% of the hair has grown back and within a year the result is 98%. This means that you are definitively freed from your hair problem!

Hair transplant is a major operation, after which there is a possibility of some complications. Some of these complications: swelling, cyanosis and puffiness in the area around the eyes, forehead, sterile (non-infectious) folliculitis, lotion or creams that your doctor will prescribe will help you quickly cope with this phenomenon, soreness and itching in the donor area will quickly disappear after use of a steroid-containing cream, your doctor recommends, numbness in the scalp. The greatest concern is a wide scar in the donor area, which may arise if the procedure of removing the grafts is performed by non-professional doctors. If hit in this area, the skin may bleed and swell. Infection can occur during or after surgery as a result of inappropriate care. Postoperative side effects should be reported to the doctor and no procedures or interventions should be performed without prior notice to the doctor.

Most doctors do not recommend returning to training earlier than a month after the operation.

Since the procedure is performed under general or local anaesthesia, the patient does not experience any pain during the operation. In the first few days after the operation, slight pain may occur, but this can be treated well with conventional painkillers.

There is no such thing as hair donation. Hair for transplantation is taken only from the patient himself, since the hair of another donor never take root.

On average, up to 90% of transplanted grafts take root. But everything strongly depends on the qualifications of the doctor and the condition of the patient. After a couple of weeks, some of the transplanted hair will fall out. This is a natural renewal, only the hairs fall out, the follicles remain in place. 100% survival rate is a myth, since 3-6% of follicles are lost even before transplantation.

The transplanted hair will grow like it's still on the back of the scalp. While most eyebrows stop growing when they are about 1 cm long or shorter, transplanted hair will keep growing.

A standard eyebrow transplant procedure typically requires 50 to 100 hair grafts, but it could be more depending on the patient's condition.

Hair loss is unpredictable. Some people find that their hairline will weaken at the age of 20, and after that it will remain stable until the age of 80. Others may have thick hair until the age of 30, and in another 5 years they will be completely bald. Because of this unpredictability, some doctors advise against having a hair transplant until the hair loss has stabilized. Usually, the complete cessation of hair loss occurs by the age of 40-45.

Transplanted hair takes root well and grows in a new place ALL LIFE. The hair taken from the back of the head retains its innate resistance to the action of hormones. Therefore, many hair transplant clinics provide a guarantee certificate.
